The discussant set the biopsy signal against the field's established local-control levers: RTOG 9408 (2yr biopsy positive 40% to 20% with 4mo ADT), ASCENDE-RT (10y local failure 7.1% to 1.5% with an LDR boost) and FLAME (crude local failure 7.7% to 2.7% with a 95Gy SIB). Each buys local control the reader can already deliver.

The nine-and-a-half-year accrual window straddles the field's move to hypofractionation, PSMA PET staging and ARSI intensification, so the control arm's 86.1-month DFS reflects an older standard. ADT use was a stratification factor but is not reported by arm in source, leaving the interaction between the viral therapy and hormonal control unresolved.

The result establishes that intraprostatic immunotherapy can reduce residual local disease at two years, which is a real mechanistic finding. What it does not settle is whether that reduction is additive to, or merely duplicative of, the local control a modern boost already delivers, and whether it converts to survival: OS and PCSM sit at one event per arm.

Aglatimagene besadenovec (CAN-2409) with radiotherapy for patients with localised prostate cancer: a phase 3, multicentre, randomised, double-blind, placebo-controlled trial
Abstract
Background About 30% of men with localised prostate cancer undergoing radiotherapy with curative intent have disease recurrence associated with progression-related symptoms and substantial toxicity of salvage therapies. Previous studies with aglatimagene besadenovec (CAN-2409, hereafter referred to as aglatimagene) showed synergy with radiation and immune-mediated cytotoxicity in patients with prostate cancer. We aimed to assess whether addition of aglatimagene plus prodrug (valacyclovir) to standard-of-care external beam radiation therapy (EBRT) could improve disease-free survival in this population. Methods We conducted a phase 3, randomised, double-blind, placebo-controlled trial at 51 medical centres (26 community and 25 institutional or military) across the USA and Puerto Rico in patients with intermediate or high-risk prostate cancer. Patients aged at least 18 years who were planning to undergo EBRT and with an Eastern Cooperative Oncology Group score of 0-2 were eligible. Patients were randomly assigned (2:1) via central block-randomisation to receive either three courses of intraprostatic aglatimagene (5 × 10 11 viral particles) plus valacyclovir or placebo plus valacyclovir, with randomisation stratified by risk category and androgen deprivation therapy (ADT) use. Patients received standard-of-care EBRT (78 Gy in 2 Gy fractions) or hypofractionated EBRT (60 Gy in 3 Gy fractions or 70 Gy in 2·5 Gy fractions) with optional ADT. The primary endpoint was disease-free survival, defined as time-from-randomisation to prostate cancer recurrence or death in the intent-to-treat population (all randomly assigned patients). Safety was assessed in all individuals who received at least one injection. The trial is registered at ClinicalTrials.gov, NCT01436968, and long-term follow-up is ongoing. Findings Between Feb 21, 2012, and Sept 9, 2021, 745 men (591 [79%] White, 121 [16%] Black) were randomly assigned to receive aglatimagene plus valacyclovir (n=496) or placebo plus valacyclovir (n=249). After a median follow-up of 50·3 months (IQR 35·2-63·3), median disease-free survival was not reached (95% CI 121·78 to not reached) in the aglatimagene plus valacyclovir group versus 86·1 (IQR 29·7-143·0) months in the placebo plus valacyclovir group (hazard ratio 0·70, 95% CI 0·52-0·94; p=0·016). Treatment-emergent adverse events (TEAEs) of grade 3 or worse occurred in 40 (8%) of 479 patients in the aglatimagene group and 17 (7%)of 232 patients in the placebo group. The most common TEAE of grade 3 or worse was acute kidney injury in both the aglatimagene group (nine [2%] of 479 patients) and the placebo group (four [2%] of 232 patients). Serious adverse events occurred in 28 (6%) of 479 patients in the aglatimagene group and 17 (7%) of 232 in the placebo group. Treatment-related serious adverse events occurred in eight (2%) patients in the aglatimagene group (four acute kidney injury, two pyrexia, and one each influenza-like symptoms and urinary retention) and five (2%) in the placebo group (four acute kidney injury, and one each increased creatinine levels and skin rash; one patient reported two serious adverse events). No treatment-related deaths were reported. Interpretation Aglatimagene plus valacyclovir was associated with longer disease-free survival than placebo plus valacyclovir when added to standard of radiotherapy for the treatment of localised prostate cancer, offering a meaningful benefit without increasing clinically significant toxicity. The longer read

The number that should move a radiation oncologist here is not the hazard ratio, it is the two-year post-treatment biopsy: 19.6% positive with aglatimagene versus 36.4% with placebo. That is the mechanism, and the DFS curve is downstream of it. Read that way, this trial belongs in a specific and crowded conversation, the one about how much residual intraprostatic disease survives definitive EBRT and what is worth adding to reduce it. The discussant made exactly that framing explicit by putting CAN-2409 on the same slide as RTOG 9408, ASCENDE-RT and FLAME, and the comparison is not flattering in the way a positive phase 3 usually is. ASCENDE-RT cut ten-year local failure from 7.1% to 1.5% with an LDR boost; FLAME cut crude local failure from 7.7% to 2.7% with a 95 Gy simultaneous integrated boost. Those are tools already in the department, already reimbursed, already familiar. The honest question is not whether the viral therapy works but whether its local-control contribution is additive to a boost or substantially overlapping with it, and this trial cannot answer that because no boost strategy was specified in either arm.

The accrual window compounds the problem. Randomisation ran from February 2012 to September 2021, nine and a half years, and the standard of care did not hold still. Conventional 78 Gy was the reference schedule at the start; moderate hypofractionation became routine during, and the protocol permitted 60 Gy in 3 Gy and 70 Gy in 2.5 Gy. PSMA PET arrived and reclassified a meaningful fraction of what was then called high-risk localised disease into occult metastatic disease. The discussant's own STAMPEDE-versus-POP-RT slide made the same point about a different trial: a high-risk cohort staged conventionally is not the high-risk cohort in front of a reader today. A control arm with 86.1-month median DFS is a plausible historical benchmark and an uncertain contemporary one, and every part of the drift runs in the direction of shrinking the measured effect if the trial were repeated now.

Two structural features deserve more weight than they usually get. ADT use was a stratification factor, which means the investigators expected it to matter, yet its distribution and its interaction with the viral therapy are not in the source. Since ADT is itself a potent reducer of two-year biopsy positivity, RTOG 9408 took it from 40% to 20% with four months, the possibility that the aglatimagene effect is largest in the ADT-free stratum and attenuated where hormonal therapy is already delivering local kill is entirely live and entirely untested here. Separately, the primary endpoint is a composite of recurrence or death, and the biopsy-driven local component is doing the work while OS and prostate-cancer-specific mortality show one PCSM event per arm. That is not a negative survival result, it is an uninformative one, and at 50.3 months in an intermediate-and-high-risk localised population it could hardly be otherwise. Long-term follow-up is ongoing, and the trial's real verdict is several years out.

What would have to be true for this to be wrong in the direction that matters? Only that the residual disease being cleared at two years is disease a modern plan would have controlled anyway, through dose escalation, a boost, or hormonal intensification. Nothing in the reported data excludes that. The safety profile, by contrast, is genuinely reassuring: grade 3 or worse events at 8% versus 7%, acute kidney injury the most common in both arms at 2%, no treatment-related deaths, so the added toxicity cost of finding out is low. That asymmetry, real local signal and low toxicity against an unproven increment over available intensification, is the honest read.
